$4,000 and $40 co-pays for "regular visits" that don't go against the
deductible, and you're well-advised to use their doctors in their
network.

They wouldn't cover pre-existing conditions for one year, and wouldn't
give me the standard rate, but offered a higher-risk-class program that
cost about 30% more.

Currently runs about $150/month for 40-44 age bracket. Escalates at
least 12% per year.
